Class B vs Class N vs Class S autoclaves: which do you need?

A Class B autoclave removes air with a fractionated pre-vacuum and is validated for every load type: solid, wrapped, porous and hollow instruments. A Class N autoclave removes air by gravity displacement and is validated only for solid, unwrapped instruments. Class S sits between them, and what it can process is defined by its manufacturer rather than by the class.
What the EN 13060 classes actually mean
Class N, Class S and Class B are not brand tiers or quality grades, and a Class B machine is not simply a better-built Class N. They are performance categories defined by EN 13060, the European standard for small steam sterilizers. A sterilizer is tested and validated against one of the three, and the class tells a buyer which loads it has been proven to sterilize.
Class N is the narrowest: solid, unwrapped instruments, used immediately after the cycle. Class B is the broadest, covering every load type the standard recognises. Class S sits in between and is defined by the manufacturer — it covers specific load types beyond Class N, and exactly which ones varies from model to model.
The real difference is how air leaves the chamber
Steam sterilizes by direct contact. Any air still trapped in the chamber, inside a wrapped pack or inside the lumen of an instrument is a pocket the steam never reaches — and that pocket is not sterilized. Everything that separates the three classes follows from this one problem.
A Class N sterilizer uses gravity displacement: incoming steam pushes the cooler, denser air downward and out of the chamber. That is sufficient for a chamber holding bare, solid instruments, but it does not guarantee complete air removal from a wrapped pack or a narrow lumen.
A Class B sterilizer uses a fractionated, or pulsed, pre-vacuum: a vacuum pump repeatedly evacuates the chamber and refills it with steam before the sterilization phase begins. Each pulse removes more of the residual air, including air held inside hollow and porous loads. That is why the same instrument can be safely processed in one machine and not the other.
Which loads each class is validated for
The standard distinguishes load types by how hard they are for steam to penetrate. Solid instruments are the easiest. Wrapped and porous loads hold air in the material itself. Hollow loads — anything with a lumen, from a dental handpiece to a suction tip to a rigid scope — are the hardest, and they are where the classes diverge.
- Solid, unwrapped
- Handled by all three classes. If this is genuinely all you process, and the instruments are used immediately, Class N is sufficient.
- Wrapped
- Required whenever instruments must stay sterile in storage rather than being used straight from the chamber. Class B; Class S only if the manufacturer validated it for wrapped loads.
- Porous
- Textiles, gowns, dressings. The material itself holds air, so pre-vacuum air removal is what makes the cycle reliable.
- Hollow / lumened
- The decisive category. If you process any instrument with a lumen, the air-removal method is not a preference — it is the requirement.
Why this is a clinical question, not a budget one
A cycle that fails to remove air does not announce itself. The load comes out hot, dry and apparently processed. Published sterilization guidance reports that bacterial endospores can survive inside hollow instruments run through non-vacuum cycles, which means an instrument can look sterile, be recorded as sterile, and not be.
This is why the class should be chosen from the loads a facility actually processes, and not from the budget or from what a comparable clinic happens to own. Buying below the load requirement does not save money; it moves an unmanaged risk onto the patient.
Choosing by facility
- Dental clinic
- Class B in practice. Handpieces are hollow instruments, and they are the routine load rather than the exception.
- Hospital CSSD
- Class B. Mixed loads, wrapped sets held in storage, and instruments that must be traceable through validation.
- Laboratory
- Depends on the load. Media and glassware differ from lumened equipment; specify by what goes in the chamber, not by the room it sits in.
- Small or field clinic
- Class N can be legitimate if the instruments are solid, unwrapped and used immediately. Be honest about whether that stays true as the clinic grows.
What procurement usually gets wrong
- Specifying by chamber size alone
- Litres are the easiest number to compare and the least informative. A larger Class N chamber still cannot process a lumened instrument.
- Ignoring cycle time at real volume
- A pre-vacuum cycle takes longer than a gravity cycle. Count the cycles a working day needs before choosing chamber size.
- Leaving out routine testing and consumables
- Validation and routine test loads, indicators, water treatment and gaskets are part of the running cost and of the audit trail. Budget them with the machine.
- No plan for installation and training
- A correctly specified sterilizer that is loaded wrongly fails in exactly the same way as an under-specified one.
